How To Apply
If you wish to begin preparing your application, you can complete the following PPP borrower application forms to apply with Pacific West Bank:

Important
If you are applying for a Second Draw PPP Loan:
- When submitting your application, include a copy of your 2019 & 2020 Quarterly Profit and Loss Statements that will be used to demonstrate your 25% decrease in revenue
- If you received your first PPP loan through PWB and the average monthly payroll amount in your new application does not match the amount calculated in your first application, please provide supporting payroll statements to justify the amount on the application.
- If the monthly payroll amount is the same as the amount used in your first application, no additional payroll documents are needed.
- If you received your first PPP loan through another bank, please provide a complete financial package to support the monthly payroll amount reported on the application.
- Items to provide include company payroll statements, State and Federal Tax Documents (Statement 940/941), 2019 Tax Returns, et cetera.
If you did not receive a PPP Loan during the first round:
- Please provide a complete financial package to support the monthly payroll amount reported on the application.
- Items to provide include company payroll statements, State and Federal Tax Documents (Statement 940/941), 2019 Tax Returns, et cetera.
Complete the application and submit all supporting documentation to Pacific West Bank. Refer to the corresponding application instructions for a listing of required documentation to submit with the PPP application. You can submit via:
A. DocuSign PowerForm - Access the FIRST DRAW Application or SECOND DRAW Application to sign electronically (links above).
B. Fillable PDF (links above) and upload to this secure ShareFile.
C. Fax the Fillable PDF (links above) to: 503-912-2091
D. Deliver the Fillable PDF to either of our branches: West Linn (2040 8th Ave) or Lake Oswego (5200 Meadows Road)
We will notify you after your application has been received and once it has been submitted to the SBA.
